Job Purpose: As a HOPE Team Outreach Specialist
at Aspire Health Partners, your job purpose will be to provide a multitude of essential functions regarding reintegration into the community of people who are homeless.
Job Functions

• Providing outreach services
• Conducting assessments and completing referrals
• Assisting clients with obtaining necessary documents to obtain housing
• Negotiating the criminal justice system and advocating for diversion when appropriate
• Adhering to contractual obligations
• Serving as acting representative to committees and community planning
• Completing required trainings and participating in required meetings
• Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
  Position qualifications: Ability to receive and maintain a(n)
  
  • Level II Background clearance
  • Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1
  • High School Diploma is required. Bachelor's Degree is preferred.
  • 1 to 2 years of experience working with targeted population may substitute for higher education.
