Job Description HOSPICE RECOGNITION PAY = ADDITIONAL 20% INCREASE ON BASE RATE THROUGH DECEMBER 31, 2020 Must undergo nicotine testing and work in nicotine-free facilities. Four eight hour days per week, to include three weekends a month. Acts as contact person with physicians, hospital discharge personnel, outside agencies, and families to assist completion of admission process. Gathers and electronically enters data related to potential patient status under the supervision of Admission RN completing assessments for possible admission to Hospice of Marion County services. Provides Hospice education and support to professional community, patient/family, and/or caregivers. ESSENTIAL JOB RESPONSIBILITIES: Include the following. Other duties may be assigned. * Cultivates and maintains relationships with discharge planners and physicians at local hospitals. * Works closely with patient/family and social work staff to facilitate discharge planning. * Demonstrates understanding of interdisciplinary team concept, cooperating and working with all team members for effective care. * Maintains up-to-date knowledge of Medicare/Medicaid benefits, Supportive Care Program, Crisis Care, and private insurance. * Maintains a good working relationship with professional community, respecting the policies and procedures of the individual facilities and agencies. * Maintains an up-to-date knowledge and understanding of local, state, and federal laws impacting patient/family care in the Hospice setting. * Possesses awareness and acceptance of various life styles without imposing personal values on the patient/family unit. * Maintains confidentiality of patient and family regarding disease/diagnosis, financial and other information obtained during the course of employment in accordance with agency and professional standards. * Uses good body mechanics when lifting or transferring patients, utilizing equipment when necessary. * Maintains universal precautions and uses necessary personal protective equipment in order to maintain a safe environment in conjunction with OSHA regulations. * Consults and collaborates with IDT to promote highest quality patient/family care. · Enters appropriate information in computer program to begin patient's medical record. · Completes Hospice consults, admission informed consent, SNF contract when appropriate, and hospice house room and board contracts, in all locations of referred patients served by Hospice of Marion County. This includes any location of patient or necessary family members i.e. private home, physician office, family member work place, SNF, ALF, hospital or clinic as requested. · Collects initial patient/family related data and completes supporting electronic documentation, incorporating psychosocial, spiritual, cultural, physical, financial and biological components. · Informs supervising RN of data related findings. · Arranges for DME deliveries with the warehouse and the patient or caregiver as needed. · Takes and verifies the doctor's medication orders for the admissions nurse as needed, documenting changes or new orders and notifying the admission nurse. · Identifies complicated needs of potential Hospice of Marion County clients during consult to assure critical DME and supplies are on hand at time of admission. · Works weekends and holidays as department needs indicate to assure available staff for admissions needs. · Complies with all Federal & State Hospice requirements and Hospice of Marion County's Policies and procedures. · Adheres to professional standards of practice for nursing. · Maintains a standard of quality care. · Allows patient to maintain quality of life to keep symptoms controlled and uses adequate documentation. · Possesses the ability to appropriately respond to and cope with patient/family emotional stress and other needs typically found in the home environment of the terminally ill. · Demonstrates tolerance to various and differing individual life styles without imposing his/her own values on the patient/family unit. · Reports pertinent changes to the Patient/Family Care Coordinator and/or supervising Admission RN. · Teaches the patient and family as required to maintain safe environment. · Actively participates in professional organizations whose goals and functions are related to those of Hospice. Maintains and up-to-date knowledge and understanding of the local, state, and federal laws impacting upon the practice of nursing in the hospice setting. · Other tasks consistent with knowledge, training and experience including hands on care/interventions with the hospice patient per hospice policy and procedures for field staff.
