Job Description:

dedicated, and skilled Registered Nurse (RN) to join our team. This
position involves providing top-quality nursing care to patients in
their homes, enabling them to achieve their highest level of
wellness in a familiar and comfortable setting. As a full-time Home
Health Registered Nurse at Aveanna Healthcare, you'll be an
integral part of our commitment to innovation and quality care,
delivering personalized health management plans and teaching
patients and their families to manage their health independently.
Conduct patient assessments, including physical, emotional,
psychosocial, environmental and educational components Develop
individualized care plans based on patient needs and physician
directions Administer medications, wound care, and numerous other
personalized interventions Direct patient care, monitoring the
patient’s overall health and prevent potential re-hospitalizations
Educate patients and their families on proper home health care
strategies and procedures Coordinate with other healthcare
professionals as needed to provide comprehensive care Documentation
of patient care services by charting in patient and department
records Strong clinical skills with a focus on
patient/family-centered nursing Excellent communication and
interpersonal skills Ability to assess patient conditions and work
with treatment plans Proficiency with electronic medical records
(EMR) Medical Insurance Dental Insurance Vision Insurance 401k
Valid RN license in the state of employment Basic Life Support
(BLS) certified Driver’s license and reliable transportation
