Job Description:
Ascension is seeking a dedicated Licensed Practice Nurse (LPN)
to join our Family Medicine Center. In line with our mission, you
will provide personalized, compassionate care, utilizing the latest
innovations in healthcare services. This role is pivotal in
enhancing patient health outcomes within our supportive and
faith-based organization. You will work directly with patients in a
family medicine setting, contributing to holistic health management
from wellness visits to chronic care management. Provide direct
nursing care and patient education to meet the health and wellness
needs of patients in a family medicine setting. Implement nursing
care plans and collaborate with the healthcare team to achieve
patient care goals. Administer medications and maintain accurate
records of patients' treatment plans and progress. Assist with
diagnostic tests and analyze results to aid in patient management
under the supervision of medical staff. Ensure the observance of
appropriate safety and hygiene standards in patient care. Strong
communication and interpersonal skills. Proficiency in clinical
skills with an emphasis on patient care and safety. Adept at
managing medical records and using healthcare databases. Ability to
perform diagnostic tests and interpret results. Medical Insurance
Dental Insurance Vision Insurance Life Insurance 401k Valid nursing
license as a Licensed Practical Nurse (LPN). CPR certification is
preferred.
