• Start & End Times of Shift: 8:00 AM - 5:00 PM (Monday to Friday)
• Dress Code: Scrubs and closed-toe shoes

  • Collaborate with providers and clinic staff to identify patients with chronic diseases.
  • Provide longitudinal care for patients with stable chronic conditions, including focused assessments, medication reconciliation, motivational interviewing, goal setting, and patient education.
  • Support patients transitioning from inpatient to outpatient care by assessing transitional needs, reconciling medications, reviewing contingency plans, and coordinating post-discharge requirements.
  • Alert providers and supervising RNs of changes in patient health conditions.
  • Document patient information accurately and promptly in electronic medical records.
  • Participate in daily huddles and Patient Care Coordination (PCC) meetings.
  • Prepare reports for weekly meetings.
  • Maintain nursing competence and knowledge of evidence-based practices.
  • Perform clinical tasks within the scope of practice as needed.
  • Handle other assigned duties.RequirementsMinimum Qualifications:
    • Active and unrestricted RN license in the state of employment.
    • Current BLS certification.
    • 2+ years of experience in a physician's office, clinical, or hospital setting.
    • Knowledge of chronic diseases like COPD/asthma, diabetes, CHF, and IHD.
    • Proficiency in electronic medical records.
    • Strong organizational, verbal, and written communication skills.
    • Effective interpersonal skills and teamwork capabilities.Preferred Qualifications:
      • Experience with patient education, motivational interviewing, and self-management goal setting.
      • IV Certification.
      • Bilingual fluency in English and Spanish (written and verbal).
